Gynaecological services include diagnostic procedure to assess the condition of the reproductive system of women. This is performed to determine if there are any infections or diseases present in the uterus, endometrium, cervix, fallopian tubes and ovaries.

When do you start seeing a Gynecologist?

Although most people won’t need to begin Pap screening for cervical cancer until around the age of 21, it is still recommended that the first gynecologist visit is earlier, usually between the ages of 13 and 16, regardless of whether menstruating or are sexually active.

There is no need to worry if this window is missed, but it is best to consider scheduling the first visit because it will be an excellent opportunity to talk about one’s development. The patient may ask any and all questions about the changing body and sexual health without hesitation.

Pelvic Scan

This is a part of a regular check-up for every woman. This scan can be performed abdominally or transvaginally to check the uterus, endometrium (lining of the uterus), cervix, fallopian tubes and ovaries. It is usually carried out as a diagnostic test in the following situations:

Tubal Patency Check

This is a special test designed to assess the patency of the fallopian tubes. It combines the latest ultrasound technology with a contrast study using a water based (foamy gel) contrast media.

This eliminates the risk of allergic reactions and causes minimal discomfort, when compared to the dye used in a standard hysterosalpingogram. It also allows us to assess the integrity of the lining of the uterus and eliminates exposure to X-ray radiation that may be harmful to the reproductive organs. It is recommended that this examination is carried out in the first half of the menstrual cycle to avoid any disturbance of a possible pregnancy. It is performed in the clinic by experienced physicians and does not require any form of anaesthesia. Although it is usually pain free, some women may experience period like discomfort during or after the examination.

Colposcopy

Screening for cervical cancer is an important part of staying healthy and avoiding cervical cancer.

The sample can easily be collected in a vaginal exam and will be sent for cytological assessment and microscopy to a special lab. The sample is taken with a special thin prep cytology brush for Liquid Based Cytology (LBC) that collects cells from the surface of the cervix (portio) as well as from the cervical channel. Therefore cells of the area of interest, the so-called transformation zone, can be examined in one test, the liquid based cytology testing.

Screening and Management of PCOS

Polycystic Ovarian Syndrome (PCOS) is one of the most common female endocrine disorders. PCOS is a heterogeneous combination of signs and symptoms of impaired glucose-insuline-homeostasis, androgen excess and ovarian dysfunction due to epigenetic and environmental influences, including diet and lifestyle factors.

Some women present with irregular periods, bloated intestines, weight gain, retention of water, hair loss or skin problems, with increased androgen levels causing hirsutism and acne. Other women are observed because they are facing problems to conceive and seek advice for conception counselling in subfertility.

The underlying pathophysiology of PCOS is a unique blend of of hyperandrogenism, insulin resistance and other factors causing follicular arrest. Multiple immature antral follicles seen in the transvaginal ultrasound imaging can be observed. The follicular maturation arrest contributes to anovulative cyles and hence causes irregular menstruation and fertility problems. Depending on the individuals’ complaint, a wide range of investigations can be offered to evaluate the extent and degree of hormonal imbalance, including laboratory tests and ultrasound examinations.

Breast Examination and Screening

Breast self-examination is a useful tool, especially when used in combination with regular physical exams and ultrasound. Sometimes additional imaging with mammography and/or MRI. Each of these screening imaging works in a different way and has strengths and weaknesses. We recommend that all women routinely perform breast self-exams.
The most common findings in a women’s breast are listed below as a brief overview

Mastopathia
This term refers to benign, meaning non-malignant structural changes in the breast tissue, that is composed of glandular tissue, fatty tissue and fibroid tissue.

Mastalgia
Pain and tenderness mostly enhanced in the second half of menstrual cycle close to bleeding time. The breasts may feel heavy, sensible, tense and firm or lumpy.

Cysts
A cyst is a non-malignant fluid filled dilated area of a glandular duct in the breast.

Fibroadenoma
A fibroadenoma is the most common finding amongst benign nodules in the breast. It most commonly affects young women as a solitary finding. 

Papilloma
A rather rare entity with 1 - 1,5% of all findings that is located intra-ductal.80% of all Papilloma show secretion of fluid from the nipple. 

Abscess & Mastitis
An acute inflammation of the breast most commonly is seen in breast feeding women consulting a doctor due to acute pain, swelling, rash and maybe fever. In the the lactating breast, inflammation occurs as a result of bacteria invasion. Tiny scars in the sore nipple area allow bacteria to affect underlaying tissue and cause infection spreading along the lactation ducts of the breast.
